An 81-year-old ENT surgeon from Ahmedabad, Dr Nandlal Manseta, offers a unique fee structure for his consultations. Patients pay based on their monthly income, with those who have no income receiving consultations free of charge. The doctor operates on a sliding-scale model, without requiring any income proof or documents from his patients.

Instead, the patient drops the payment into a small donation box at the clinic. The fee chart displayed at the clinic sets the consultation cost according to the patient's income; for instance, a monthly income of Rs 1,000 results in a Rs 10 fee, while a Rs 10,000 monthly income corresponds to a Rs 100 fee. Manseta's approach stems from his desire to make medical care accessible to all, regardless of a patient's financial situation.
