Investigations underway into surgery mix-up at Melbourne hospital

An investigation is underway in Melbourne after a woman received a surgery intended for another patient at a hospital in the city's west. The incident occurred at Joan Kirner Women's & Children's Hospital in Sunshine in January. The woman, Debra Buchanan, had traveled to the hospital for a minor procedure to remove a lesion on her pelvic area. However, she awoke from surgery to discover a portion of external tissue from her genital area had been removed, resulting in permanent scarring.

Ms Buchanan's lawyer, Erin Monsalve Fear, stated that the mistake constituted a "complete systemic failure" on the part of the hospital. She explained that there are numerous safeguards and procedures in place to prevent such occurrences, and it is evident that these were not followed in Buchanan's case. The error left Buchanan with physical pain, permanent disfigurement, and a loss of trust in the medical profession.

She has now required a second surgery to address the original cancerous lesion she was initially supposed to undergo surgery for.

The government has committed to being open and transparent during the investigation. Safer Care Victoria, the agency responsible for overseeing patient care in the state, is reviewing the incident. Western Health, the hospital where the mistake occurred, acknowledged the error but declined to comment on individual patient care due to privacy and confidentiality obligations.

They stated that they would provide an apology, meet with the patient, conduct a review of the incident, and share findings to prevent future recurrences. Despite the surgery taking place in January, Western Health has yet to apologize to Buchanan.
