A Toyota Yaris and a Yamaha Y15 motorcycle collided in Kampung Teluk Sareh, near Kampung Gajah, resulting in the fatalities. The disaster management team received a report around 4:29 AM, prompting a team from the Gajah Village Bombing Command to mobilize to the scene.

Upon arrival, the disaster management team found the Toyota Yaris trapped in a ditch with one fatality inside, while the two passengers of the Yamaha motorcycle were also trapped in the ditch. Both the motorcycle rider and passenger were buried in the mud, according to the disaster management team. The two deceased victims, Siti Qazrina Abdul Rahman, 19, and her brother Adam Danial Abdul Rahman, 18, were pronounced dead at the scene by medical officers.

The deceased victims were handed over to the police for further investigation, while the injured driver of the Toyota Yaris was taken to Hospital Seri Iskandar for further treatment.
