Simultaneously, a Proton Persona, driven by a 20-year-old man with three passengers, was traveling from Jeli towards Gerik, while a Hino lorry, operated by a 42-year-old man with a 34-year-old male attendant, was also heading in the same direction. Police stated that the Perodua Axia likely went out of control, entered the opposite lane, and collided with the rear of the Proton Persona before striking the front of the Hino lorry.
The collision caused the Perodua Axia to become lodged beneath the lorry, dragging it a distance of three to seven meters. Four occupants of the Perodua Axia perished at the scene, while one child sustained serious injuries. The driver and passengers of the Proton Persona, as well as the lorry driver and attendant, were uninjured.
