Woman's 5-year struggle with balance and walking was caused by a 'one-in-a-million' disorder

A 50-year-old woman from New York endured a five-year battle with balance and walking issues due to an exceptionally rare disorder known as stiff person syndrome (SPS). The patient initially sought medical attention at another hospital a month prior, complaining of persistent back pain, knee stiffness, fever, and substantial weight loss. However, she revealed that her discomfort had been escalating for five years. Gradual rigidity in her right leg caused her frequent falls and affected her balance.

Upon her first hospital visit, blood tests showed normal white blood cell count, ruling out infection. Her blood tests also did not indicate rheumatoid arthritis, HIV, Lyme disease, or Sjögren's syndrome, which can cause similar symptoms. Despite this, her symptoms worsened, eventually leaving her bedridden and unable to sit or turn. Doctors noted her leg stiffness prevented testing her lower extremity range of motion.

Upon further examination, doctors identified the telltale sign of SPS: elevated levels of antibodies blocking the enzyme glutamic acid decarboxylase (GAD). This blockage prevents the production of GABA, a chemical messenger that helps control nerve cell activity. Without sufficient GABA, nerve cells controlling muscle movement can become overactive, leading to spasms and preventing muscles from relaxing.

The patient was treated with intravenous immune globulin, rituximab, prednisone, gabapentin, diazepam, physical therapy, and ongoing monitoring. Within days, her condition improved, and she was able to turn in her bed, flex her hips and knees, stand with assistance, and finally walk using a rolling walker three months after discharge.
