Based on this assessment, a personalized action plan is created, which the veteran can approve through the Diia application.
The support process is structured into five key stages: discharge preparation, civilian life transition, health recovery and psychological support, economic independence and professional fulfillment, and socialization and community involvement. The Ministry emphasized that performance will be measured not by the number of consultations or referrals issued, but by the actual improvements in veterans' well-being and their families'.
To ensure effective service delivery, the ministry plans to overhaul the workload assessment model for support specialists. Instead of allowing a single specialist to manage up to 100 cases, a point-based system will be implemented to account for the complexity of each individual case, ensuring more tailored and impactful support.
