Staff slept while patient killed at NHS mental health unit

An NHS mental health unit in east London witnessed a fatal attack on a patient, Hugo Flint-Cahan, by another individual, Rolando Torres-Pena. The inquest revealed that staff had falsified records, slept, and were focused on their phones during the incident. The inpatient unit at Newham Mental Health Centre, run by the East London NHS Trust, was understaffed, with only two nurses and a nursing assistant present on the night of the tragedy.

Torres-Pena had been transferred to the ward five days prior to the attack. CCTV footage revealed that Hugo was wandering the corridor, while Torres-Pena was pacing the corridor, also unsettled. Staff failed to check on patients regularly, and the observation log incorrectly stated that Hugo was awake at a certain time. The attack occurred around 01:26, and Hugo was discovered dead at 03:19 by a nursing assistant who had taken an unauthorized two-hour break.

The coroner concluded that Hugo's death was unlawful and contributed to by neglect. The inquest highlighted repeated failures by the Trust, including falsified records, poor communication, and inadequate patient assessments. The Trust has acknowledged some of the issues and is launching an investigation into the staff involved. Hugo's family expressed their shock at the dangerous failures in his care and claimed that his death was preventable with proper care.
