Varicose veins, a common manifestation of chronic venous insufficiency, have long been associated with inflammation. However, the specific immune cell populations and mechanisms involved have been elusive. To investigate, researchers subjected varicose veins to a stepwise mechanical and enzymatic dissociation, isolating mononuclear cells from the connective tissue and vein wall.

These cells were then characterized using flow cytometry, revealing two distinct populations of macrophages: CD206-negative IL-1+ S100A8/9+ macrophages in the connective tissue and CD206-positive TIMD4+ LYVE1+ macrophages in the vein wall. The T cells were predominantly found within the connective tissue, exhibiting an effector-memory T cell phenotype and producing IFN-gamma in situ.

Upon analyzing the cytokines produced by the macrophages, researchers discovered that the combined effect of these cytokines could induce IFN-gamma, TNF-, IL-22, and GM-CSF in effector memory T cells. Additionally, the study found that TNF- and IL-10 were key drivers of this process. The findings suggest that the identified macrophage-T cell cytokine axis plays a significant role in the pathogenesis of chronic venous insufficiency and may serve as a potential therapeutic target.
