Scientists have developed a new radiotheranostic pair called 68Ga/177Lu-NYM096 that can detect and treat metastatic clear cell renal cell carcinoma (ccRCC). This innovative approach combines diagnostic and therapeutic radioisotopes in a small-molecule compound, targeting the overexpressed carbonic anhydrase IX (CAIX) enzyme in ccRCC.

The research, published in The Journal of Nuclear Medicine, offers a safer and more precise treatment strategy for patients with end-stage ccRCC, a subtype accounting for 70% to 80% of all renal cancer cases.

Preclinical and first-in-human experiments demonstrated high tumor accumulation of the radiotheranostic pair in ccRCC tumor-bearing mice, with 177Lu-NYM096 showing a significant dose-dependent tumor suppression effect. In patients with metastatic ccRCC who had disease progression after standard therapy, 68Ga-NYM096 PET/CT imaging demonstrated excellent tumor uptake, while 177Lu-NYM096 treatment exhibited no evidence of nephrotoxicity, hepatotoxicity, or pancreatic toxicity. However, gastric toxicity was observed.

The imaging agent identified tumors with very high uptake, and early clinical data suggested that targeted radiotherapy could provide a new treatment strategy for selected patients.
