"summary": "Tennessee's prisons chief, Frank Strada, has resigned following a botched execution attempt on death row inmate Christa Pike. Pike, 50, was left in critical condition, unconscious, and on a ventilator in a hospital after two lethal injections of pentobarbital failed to kill her on Wednesday.\n\nThe incident, which occurred at Riverbend Maximum Security Institution in Nashville, prompted Tennessee Governor Bill Lee to halt attempts to carry out the death penalty for the rest of 2026 and order an independent review of the execution attempt. According to Governor Lee, Strada will leave his position this month and an interim commissioner will be announced later.\n\nStrada stated that stepping aside is in the best interest of Tennesseans as the independent review moves forward, while also asserting that his agency's personnel followed proper protocol. This is the state's second failed execution attempt this year, with Pike's lawyers accusing Correction Department officials of ignoring prior warnings.",
